The Healthcare Power of Attorney session will be held on Tuesday, Aug. 13 at 7:00 pm in the Brown’s Chapel Fellowship Hall. Mr. Messick, an attorney in Pittsboro, may say a few words at first and then we will have 3 lines with a notary and 2 witnesses for each form. Please do not get in a line with a witness or notary to whom you are related.
If you are coming, please bring a valid ID and print (from this site) or get a printed copy of the form at church on Sunday and review. You will need one copy for each person for whom the form is being completed and then need to fill in section 1 on page 1 with name and the people to be assigned as health care agents. It is important to get their agreement to be the health care agent before you complete the form.
Check sections 5 and 6 to see if you would like to make any additions. You should not sign at the end of section 9 until Tuesday night when you are in front of witnesses and notary. After each form is complete, you will be responsible for keeping the completed form and making copies to keep in an accessible place or leave with your doctor, health care agents and anyone else you think should have a copy. There is no charge for this service and we hope everyone will take advantage if you do not have a form.
